BBG RULES & GUIDELINES AGREEMENT
Entertainment studio rules are for the benefit of all students. We emphasize the consistent practice of dance, acting and communication classes, performance etiquette and dress code to help students become the best they can be.
ATTENDANCE:
All payments for BBG packages, classes and special events must be paid for before students can participate.
2. Students must arrive 5-10 minutes before class to allow time to use the bathroom, get dressed, remove jewelry, and neatly put their hair back.
3. All students MUST be picked up within 10 minutes after class.
4. Students who arrive late may only begin class with permission from the director/teacher. Dance instructors may ask students to do class warm up before starting dance class.
5. Students are not allowed to miss classes or scheduled rehearsal during the program. Please speak with BBG STAFF to discuss in previously scheduled obligations prior to starting class.
STUDIO/CLASS RULES:
6. Students must be dressed properly and have their hair tied up neatly.
7. Students and parents are responsible for their belongings. (Do not bring valuables into the studio or waiting area if it’s not a necessity. BBG is not responsible for lost, stolen, or misplaced property.)
8. No chewing gum or chewing food during classes! We do not want anyone choking during class. There are designated times and areas where students will be able to eat.
9. Please no open soda cans or containers anywhere in the studio. Students are responsible for cleaning up after themselves.
10. Always bring a water bottle to class.
11. No parents/caregivers/siblings/friends will be allowed in the studio while class is in session.
12. No loud noises allowed in the waiting area when class is in session.
13. No photography or videotaping allowed at BBG performances or in the studio. without authorization from Butts Boulevard Global Inc.
14. ALWAYS COME PREPARED TO WORK, AND GIVE 100% EFFORT DURING ALL CLASSES!

AGREEMENT, RELEASE FROM LIABILITY, WAIVER OF CLAIMS & ASSUMPTION OF RISK
PARENT/LEGAL GUARDIAN WILL BE REPRESENTED AS “I” IN THIS LEGALLY BINDING DOCUMENT
I agree to pay all class fees for the program I am enrolling my child in at Butts Boulevard Global Inc. I, on behalf of myself, my heirs, executors, agents, assigns, and representatives, hereby indemnify, release and forever hold harmless Butts Boulevard Global Inc. as well as its executives, directors, employees, teachers, volunteers, assistants, associates, and instructors, from any and all claims of liability arising from any accident, personal injury, death, personal property loss or damages sustained by my child, while participating in Butts Boulevard Global Inc. classes or events at Butts Boulevard Global Inc., or outside performance opportunities representing Butts Boulevard Global Inc.
I understand that dance activities have inherent risks of injury, and, being fully aware of all risk, consent to my child’s participation and accept full responsibility for providing adequate health and accident insurance coverage for the protection of my child. By signing this statement, I declare that my child, who I am enrolling at Butts Boulevard Global Inc. is in good health, with no physical conditions that might prevent participation in strenuous and rigorous dance activities and other training and performance connected to classes or performance opportunities. Further, I understand and acknowledge that because of the physical nature of DANCE, there may be physical contact between teachers, instructors and students during class, and especially during dance instruction. I understand that at times for proper instruction and safety, physical contact is required and necessary. I have carefully read this Agreement, Waiver, Release, & Assumption of Risk and fully understand its contents.
I understand that this is an assumption of risk and release of liability, and I sign it of my own free will. I also authorize Butts Boulevard Global Inc. to use photos and videos of classes, events, or outside performances for promotional purposes and provide my legal consent to do so.
I acknowledge that as the parent or legal Guardian of the child I am enrolling for class (at my own free will) at Butts Boulevard Global Inc. I understand and accept the terms of this agreement.

*ALL Butts Boulevard Global Inc. programs are dedicated to developing students who seek to become professional entertainers, and lead a life of excellence on and off the stage.
While ALL programs are performance based, ALL students are REQUIRED to give 100% effort at ALL TIMES!
Additionally, skill set is only ONE of the many components we evaluate students on. Programming is designed to help students who are COMMITTED TO SUCCESS. We EXPECT STUDENTS TO BE FOCUSED, POSITIVE, PASSIONATE, SUPPORTIVE PEERS, and RELENTLESS about perfecting their craft and themselves.
If ANY student displays a lack of willingness to commit to this program their registration may NOT be accepted for Spring Semester. Students unwilling to commit to this level, may also be pulled from scheduled or unscheduled performance opportunities.
We also expect parents/legal guardians to support students, and be open to a FLEXIBLE schedule, as new performance opportunities will arrive. If ANY student, parents/guardian has reservations about committing to these terms, we suggest you speak with BBG Staff to determine if this program is a right fit for your child's needs.
